Payment
Payment options include cash, check, credit and debit, Venmo, PayPal, Health Savings Account (HSA) cards, and gift certificates. As always, small businesses value and appreciate cash payments. Although gratuities are always welcome, our high standard of care will never be compromised.
Insurance
Motor Vehicle Accident (MVA) and Personal Injury Protection (PIP) insurance are gladly accepted. In order to bill, please be prepared to provide us with your claim number, date of accident, and a referring provider.
Your private or commercial health insurance may cover Flourish services. If your insurance benefits package includes massage therapy/manual therapy provided by an out-of-network provider, your Flourish services are likely covered by your insurance policy. To confirm, please check your benefits package, deductible, and co-pay specifics prior to scheduling your appointment. If a co-pay is required, it will be collected at the time of your Flourish service. If you're unsure whether your insurance will cover an already-scheduled Flourish service, please come prepared to pay out-of-pocket for your visit and until your benefits are confirmed. You are responsible for any Flourish service costs or claims that are not covered by your insurance.
If you would like to submit coverage claims directly to your insurance company, we are happy to provide you with an itemized "superbill" for your Flourish services.
Regrettably, we do not accept Oregon Health Plan (OHP) coverage.
Appointment Protocols
When possible, first time clients should complete and submit our online intake form in advance of their appointment. Otherwise, please arrive 10 minutes prior to your appointment time to do so in hard copy. To ensure fully-allotted time in your session, all clients should arrive a few minutes prior to their appointment time. Unfortunately, late arrival can and will not change your appointment end time. Upon arrival, remember that there are often other sessions in progress. Please enter quietly, remove your shoes, and silence any mobile devices.
The personal privacy of your massage or yoga session and associated details is among our highest priorities and will be kept in compliance with relevant HIPPA regulations as acknowledged by your signature on our intake form.
We offer highly-skilled, professional and therapeutic massage and yoga services. Comments, suggestions, or requests of any other nature will not be tolerated and may result in nonrefundable termination of an appointment.
Cancellation
Refundable cancellations are accepted up to 24 hours before your scheduled appointment. After that time or in no-show cases, you will be charged a cancellation fee equal to 50% of your originally scheduled service.
