I understand that I am responsible for any Flourish service costs or claims that are not covered by my insurance (if any).*

I understand that refundable cancellations are accepted up to 24 hours before my scheduled appointment. After that time, or if I miss my appointment, I will pay a cancellation fee equal to 50% of my originally scheduled service.*

I understand that I will be receiving highly-skilled, professional, and therapeutic massage or yoga services only. Comments, suggestions, or requests of any other nature will not be tolerated and may result in nonrefundable termination of an appointment and denial of future services.*
